Speaking from nigh-personal experience (i.e. my wife), the problem is entirely that the government has a monopoly on residencies. It is simply impossible to become a practicing doctor without completing a residency, so it is somewhat inaccurate to refer to them as a government subsidy. Although technically true, the more fundamental point is that residency is a government *control*.

Internship itself has to be removed from the hands of the government. Any physician has to be allowed to teach medicine, to have as many interns they want.
